Home Care Nurse

Key Responsibilities:

• Provide direct patient care in a home environment under the supervision of a Registered Nurse or physician.
• Assist patients with activities of daily living (ADLs), including bathing, dressing, feeding, and mobility.
• Monitor and document vital signs such as temperature, pulse, respiration, and blood pressure.
• Accurately administer prescribed medications and monitor patients for side effects or adverse reactions.
• Perform personal hygiene, wound care, and diagnostic support procedures per standard protocols.
• Initiate and manage IV therapy, monitor infusion flow rates, change dressings, and discontinue IVs and blood products as needed.
• Respond promptly to emergencies (e.g., choking, CPR, fire) and provide appropriate support.
• Observe patients and report significant changes in condition to the supervising RN or physician.
• Educate patients and their families on treatment plans, medications, and health management practices.
• Maintain thorough, accurate, and confidential documentation of all patient care activities.
• Collaborate effectively with a multidisciplinary healthcare team to implement personalized care plans.
• Uphold professional nursing standards and participate in ongoing training and development.

Qualifications and Requirements:

• Completion of an accredited nursing program.
• Valid nursing license from the Nursing and Midwifery Council of Ghana.
• Minimum 2 years of nursing experience, preferably in home care or community health settings.
• Current CPR and First Aid certification.
• Strong clinical assessment and critical thinking skills.
• Compassionate, patient-focused approach with excellent interpersonal skills.
• Ability to work independently, manage time effectively, and respond to emergencies calmly and efficiently.
• Proficiency in documenting care and using digital record-keeping systems is an advantage.
• Must be comfortable working in varied home environments and traveling within assigned areas.
