Registered Nurse - Rn Case Manager

Join our team as an RN-Case Manager, providing in-home care to former Nuclear Weapons Workers with chronic or terminal illnesses caused by exposure to toxic substances.
- Responsibilities: Conduct weekly in-home nursing visits, perform physical assessments, ensure home safety, manage medication planning, oversee caregivers, and coordinate client care.
- Qualifications: Graduate of a state-approved nursing program, current RN license, minimum two years nursing experience including one year in home care, CPR certification, and preferred BSN and supervisory experience.
- Skills & Duties: Provide direct patient care, collaborate with team members, develop and evaluate care plans, ensure policy adherence, participate in staff training, review clinical documentation, and support performance improvement initiatives.
- Additional tasks: Assist in staffing, participate in interviews, contribute to marketing efforts, and perform supervisory visits as needed.
