- Work collaboratively with internal and external stakeholders to provide a coordinated response to children and young people with a disability at risk of, or requiring, supports outside the family home; working with service providers and families to ensure the best interests of the child are addressed through their care arrangements.
- Support the establishment of accommodation arrangements for children and young people who are unable to live in the family home due to their complex disability needs, including monitoring and oversight of the board and lodging arrangements in accordance with relevant policy and operating frameworks for children and young people with a disability.
- Provide mainstream case coordination support, according to individual circumstances, to ensure children and young people in care arrangements can either return to living with their families or smoothly transition into adult services which meet their long-term needs.
